The 16-year-old was last seen in Flemington Road, Parkville at 5:00 pm on August 27.
Jordonn was last seen wearing a black Fila pullover, red tee shirt, navy tracksuit pants with grey/white stripes on the side and black runners.
He is described as 160 centimetres tall, slim build with short shaved hair with a line on the left side, a rat’s tail and has both ears pierced.
Police and family have concerns for Jordonn’s welfare due to a medical condition.
He is known to frequent the Altona area.
Police have released an image of Jordonn in the hope someone knows of his current whereabouts.
