The visitors took the lead in the 22nd minute through Jonathan Rowe’s powerful strike from outside the box.
Evan Ndicka made in 1-1 shortly after when he headed home a corner taken by Lorenzo Pellegrini.
Just before halftime, bologna retook the lead as Federico Bernadeschi converted a penalty awarded for a clumsy challenge on Nadir Zortea.
Santiago Castro made it 3-1 on the night in the 58th minute with a thunderous strike that left Roma goalkeeper Mile Svilar with no chance.
Donyell Malen got one back for the hosts with a penalty awarded for a foul on Robinio Vaz.
Pellegrini scored Roma’s equaliser in the 80th minute with a strike into the bottom corner from the edge of the box.
With the game heading into extra time, Nicolo Cambiaghi snatched the winner in the 111th minute with a low shot into the near post that went in off the upright.
Next up for Bologna is a two-legged quarterfinal against English Premier League side Aston Villa.
