Sassuolo scored after seven minutes thanks to a Martin Erlic header and were managing well even after a red card for defender Ruan Tressoldi 17 minutes into the second half.
But added time proved action-packed.
Seconds after Sassuolo had a second goal ruled out for offside, Gianluca Lapadula equalised for the hosts.
Then, with almost the last kick of the game, Leonardo Pavoletti scored from an overhead kick to put Cagliari ahead and give the Sardinian side only their third win of the season.
The win takes them out of the relegation zone, while Sassuolo are 15th.
