La Viola were dominant, with 73 per cent possession and 23 chances created. Plzen did not have a single shot on goal.
At the end of regulation time, the score over two legs was still 0-0, frustratingly so for the home side.
But under two minutes into extra time, Nicolas Gonzalez made the breakthrough Fiorentina so desperately deserved with a half volley resulting from a corner.
Cristiano Biraghi sealed the deal in the 108th minute when he finished off a counterattack mostly carried out by Jonathan Ikone.
Fiorentina next face Club Brugge with the first leg kicking off at 5 a.m. on Friday, May 3 (AEST).
