Di Lauro, 38, is the fourth son of former Camorra boss Paolo Di Lauro, who was at the helm of the clan until his arrest in 2005.
“Unusual activity” led authorities to the fugitive’s apartment in the Chiaiano district of Naples, where he lived with his wife.
He was sitting with his two cats and eating pasta when police arrested him without a fight, in an operation involving 150 officers.
Police found no weapons and only a small sum of money in the apartment.
Italian media reported that the arrest was connected to a murder that happened earlier on Saturday that was carried out by a man linked to Di Lauro.
Authorities have not confirmed whether the two incidents are related.
Interior Minister Matteo Salvini on Saturday congratulated authorities on completing a “very important mission”.
According to the Interior Ministry, Di Lauro was among the top four most-wanted criminals in Italy after the escaped a massive police raid in 2004.
Di Lauro was responsible for at least four murders, an informant told authorities in 2010.
He was considered to be the second-most dangerous man in Italy after Sicilian Mafia boss Matteo Messina Denaro.
